1. What is Endometriosis Surgery?

Endometriosis occurs when tissue similar to the lining of the uterus grows outside the uterus. Surgery may remove these deposits, release adhesions and treat associated cysts or other affected tissues.

Surgery may be considered when endometriosis causes persistent pelvic pain, painful periods, pain during intercourse, infertility, endometriotic cysts or symptoms that do not respond adequately to medical treatment.

Common symptoms include severe menstrual pain, chronic pelvic pain, painful intercourse, painful bowel movements or urination during menstruation, heavy or irregular periods and difficulty becoming pregnant.

The surgeon generally uses a laparoscope inserted through small abdominal incisions to identify and treat endometriosis. Treatment may involve excision of endometriosis, removal of endometriotic cysts and division of adhesions.

Surgical options include laparoscopic excision or ablation of endometriosis, ovarian endometrioma surgery and adhesiolysis. The appropriate technique depends on the location and severity of disease. Hormonal treatment may also be used before or after surgery when appropriate.

Laparoscopic endometriosis surgery commonly takes around 1–3 hours, although extensive disease can require longer surgery. Many patients return home the same day or after one night, while recovery commonly takes 2–6 weeks, depending on the extent of surgery.

Surgery can improve pain and, in selected patients, fertility outcomes, but endometriosis can recur after treatment. Possible surgical complications include bleeding, infection, adhesions and injury to nearby organs such as the bowel, bladder or ureters, particularly in extensive disease.
